No official policy. Mutual agreement between theses advisors and students has worked well.

Is there family health insurance for graduate students? If so, is it included in the stipend?

Student insurance is covered by the university, addition family membership paid by student. It is not included in the stipend.

Please describe why someone applying to graduate school who is interested in a female-friendly department should choose your department.

There has been a history of female graduate students working with all faculty members in the department. Following is a partial list of female Phd's from different groups in the last few years:

Condensed Matter Experiment: Kirstin Purdy, now Postdoc at University of Illinois-Urbana-Champagne.
Condensed Matter theory: Allison Ferguson, now postdoc at U. Toronto, Silke Henkes, now postdoc at Leiden.
Neuroscience: Caroline Geisler, now Postdoc at Rutgers.
Astrophysics: Leslie Brown, now Professor and Chair, Connecticut College.
High Energy Experiment: Dana Partos, now working in industry.
High Energy Theory: Marci Knutt, Research Associate, McGill University, Montreal, Quebec, Canada.

Brandeis collaborates with 4-year women's colleges in the area, which promotes interaction with other female faculty and students:

Yue Hu (Wellesley)
Nalini Easwar (Smith)
Shubha Tiwari (Mount Holyoke)

Female students have expressed a preference for a small department like ours which encourages discourse between faculty and students.
