We have a general leave policy that graduate students may take a leave of absence from the program for any personal reason.

Is there family health insurance for graduate students? If so, is it included in the stipend?

We do have family health insurance for graduate students. Health insurance is not subsidized separately from the stipend.

Please describe why someone applying to graduate school who is interested in a female-friendly department should choose your department.

Female graduate students benefit from the fact that diversity of both faculty and students is a major focus of Carnegie Mellon's official strategic plan. Our standing Diversity Advisory Council monitors all diversity issues and publishes a yearly report on progress together with recommendations for further steps to be taken.
We have two major programs that focus on developing a female-friendly environment for graduate students. Our Graduate Women's Gathering, under the direction of the Graduate Programs Office, runs a calendar of over 10 events/year addressing issues of interest to female graduate students. Carnegie Mellon's Women in Science Group is centered in the Mellon College of Science which contains Physics as one of its four departments. It runs a similar calendar of programs which are more focused on female graduate students in the sciences.
The Physics Department strives to provide all students with a friendly and supportive environment. We offer multiple avenues for every Physics student to seek help with any sort of problem they encounter during their graduate training. Students often comment on the openness of the Physics Department and how approachable all faculty is. We make every effort to insure that every student feels welcome in the Department.
